Various support initiatives need to be put in place in order that the knowledgeable and adequately-equipped health-care provider is aided in diagnosis according to internationally-agreed criteria and in management according to locally-accepted guidelines. The use within these guidelines of appropriate and cost-effective drugs, and the avoidance of misuse, are both keys to success in providing care.

Iterative follow-up, with outcome review, is the only way to ensure that best possible outcome is achieved. Validated measures of outcome are needed that respect patients' views of what best outcome should be. These have to be devised and agreed.
